As you plan events as a Ministry Team, organize a special community event, or host a seminar, we ask you to contact the church office to talk through which type of form to fill out so we can make sure the space is ready for you on the day of your event. We do not make those forms available on our website since we want to make sure you use the correct form. Simply call or email the church office and let us know what you are planning and we'll send the form to you.

You may fill out this online membership form to start the membership process for Bethel Church. If you would like a hard copy of the form, please contact the church office and we will be happy to mail it to you.

This form is an opportunity to let Bethel Pastors know that you or your child are interested in being baptized.

Are you interested in having your wedding at Bethel Church? The first step is to fill out and return this request form.
Once your specific requests are reviewed, we will set up an appointment to meet with you and your fiancé. You will be given a brief overview of weddings at Bethel, a tour of the facility and information on various fees.

Funerals are planned and organized through the funeral director of the funeral home. Bethel has no forms that you will need to fill out.
